1) Review your referrals over the past year. What procedures have you referred out most? Evaluate whether or not it will be more profitable to perform any of those procedure yourself. A Holter Monitor averaging $9,000 is as perfect example, if you are privileged to perform these test. Medicare will pay a Miami provider about $154 for this procedure. If a provider in that area bills this procedure just 5 times per month, the monitor will pay for itself in less than a year.

2) How about your patients that you have urged to lose weight for the benefits of their health. Do you think trying to convince them to lose weight is enough? How about adding a nutritionist to your staff? It will be so much more beneficial, if after seeing you, a patient can be seen by the nutritionist on staff. A patient will more likely to work on their weight loss plan if they know that they are going to be seen by their doctor and their nutritionist the same day.

3) How about yoga and/or aerobic classes? If you have the space this can be a great addition. You can start out with one or two one hour classes per week and add more as you go along. Some patients get a sense of comfort going to their doctor's office for these classes.

4) Adding a Physician Assistant (PA) to your practice can create another income stream. When most patients are sick, they do not care if they are seen by you or your PA. Remember, PAs are formally trained to provide diagnostic, therapeutic, and preventive health care services, as long as they are being supervised by you. They may also prescribe some medications.

5) How about establishing hours to accommodate your patients. This does not mean increasing your hours, but instead changing them for more flexibility. For example, you can hire a part-time provider to work from 8 am to 1 pm, while you work from 12 noon to 8 pm. In addition, your Physician Assistant could work from 10 am to 7 pm. This will give your patients the opportunity to be seen before work, after work or during lunch, whenever convenient to them. 6) Do your research to see if adding an ancillary service will increase your revenue? Sometimes it does, other times it is a financial disaster. Calculate whether or not, it will pay for itself. Consider a service that is not covered by any self-referral regulations. Make sure you consult a healthcare attorney for details on Federal and State compliance and regulations.
